Position Summary
The ADHC Social Worker is responsible for ensuring that the department and all staff under its direction provide intake assessments and patient advocacy services, and for coordinating social work–related matters among physicians, staff, participants/patients, families, and other agencies.
The Social Worker works to reduce or eliminate barriers that interfere with participants’ access to health care and utilization of medical and rehabilitative services.
These activities include serving as a resource to licensed professionals and other service providers in the following areas:

Access to community resources

Arranging necessary medical equipment or supplies to support clients’ return home

Making appropriate referrals to state and government agencies for identified issues

Referrals to local self-help and support organizations

Representing ADHC at meetings that integrate the program with community health, education, and social service agencies

Providing individual and group counseling in accordance with each participant’s Individual Plan of Care (IPC)
Minimum Qualifications

Master’s degree in Social Work (MSW); or
Master’s degree in Psychology, Gerontology, Counseling, or a related field with one (1) year of relevant experience; or
Bachelor’s degree in Social Work (BSW) with two (2) years of relevant experience (e.g., geriatrics, health care, long-term care services)

Bilingual proficiency in Korean and English

Ability to work effectively with individuals with physical and/or mental limitations
Essential Functions / Responsibilities

Conduct various assessments, develop treatment plans, and provide participant advocacy

Research and coordinate community resources

Monitor and report case documentation in a timely manner

Participate in or lead coordination of social and health services

Attend team meetings and provide consultation to therapists, families, and participants

Support the Program Director with daily ADHC operations

Assist with staff supervision, audit preparation, and reporting

Perform other duties as assigned
Working Conditions / Physical Requirements
This position is primarily performed in an office setting. The role may occasionally require bending, reaching, lifting, or moving office supplies weighing up to 25 pounds (approximately 11 kg). Daily use of a company-provided computer is required to input, review, and edit text and graphics via computer terminals and documents. Occasional travel to attend internal and external meetings is also required.
